Story

My name is Serena Basten Kachinsky, and I bring over two decades of bedside experience in emergency departments across the country to my midwifery practice. Most of my 20+ years were spent as a registered nurse, where I honed my skills and deepened my commitment to compassionate patient care.
My journey to midwifery is the realization of a lifelong dream inspired by my childhood. As a young girl, I idolized my aunt, a practicing OB/GYN, and spent countless hours pretending to “catch babies.” Baby-catching is in my blood; my paternal great-grandmother was a midwife in Appalachia, a legacy I proudly continue.
For me, midwifery is more than a profession; it is a calling to give meaning to life by supporting one of its most profound and universal experiences: birth. I believe every family deserves a compassionate and empowering journey into parenthood, rooted in respect and care.

Personal Experience
My personal birth experiences on The Farm in Summertown, TN, where I delivered my three children, were pivotal in shaping my passion for midwifery. The birth of my daughter, Claire Aspen, reignited her desire to support families through the transformative process of pregnancy, birth, and postpartum care.
While I cherished my career as an emergency department nurse, I am thrilled to now guide families through their own birthing journeys.
A firm believer in the power of physiological birth, I advocate for creating supportive, intervention-free environments where the natural process of birth can unfold safely.
I am also committed to working as an integrated part of the medical system, ensuring my clients receive evidence-based, culturally sensitive care.
I empower families with knowledge, resources, and data, allowing them to make informed decisions that align with their beliefs and values.



My dedication to culturally sensitive care stems from my diverse experiences, including working with the Amish community and the multicultural population I served at Kaiser Oakland.
I am a fierce advocate for informed consent and client autonomy, empowering my clients to feel safe, respected, and supported throughout their care.




Background, Training, & Experience
I completed my midwifery training with The Farm Midwives in Summertown, TN, which is world-renowned for its expertise in physiologic birth. I achieved my certification through the rigorous NARM PEP process and continue to prioritize professional growth by maintaining over 45 hours of continuing education annually in topics such as fetal monitoring and obstetric emergencies.
I am certified in Neonatal Resuscitation, Basic Life Support, Pediatric Advanced Life Support, and Advanced Cardiovascular Life Support (all of which are renewed every 2 years). I also actively participate in local peer review with Tennessee midwives to ensure accountability and excellence in my practice.
I hold a biology degree, magna cum laude with highest honors, from Sweet Briar College in Virginia, and earned my nursing degree from the College of the Redwoods in Humboldt County, CA. In addition to my midwifery work, I am a trained Sexual Assault Nurse Examiner (SANE), serving victims of sexual violence in southern Middle Tennessee through the Center of Hope.
Modwella Services
In addition to my work as a midwife, I also serve as a nurse with Modwella, providing in-home infusion and injection services throughout Middle Tennessee.
I collaborate with midwives, birth centers, physicians, nurse practitioners, and other healthcare providers to help clients receive timely treatment in the comfort of their own homes. Services include iron infusions, IV hydration, vitamin infusions, injectable vitamins, and other wellness-focused therapies designed to support optimal health during pregnancy, postpartum, and beyond.
I also carry commonly prescribed medications for nausea, pain management, and other acute concerns, with access to licensed providers who can evaluate and prescribe treatments when appropriate. Whether you’re struggling with iron deficiency, dehydration, pregnancy-related nausea, postpartum recovery, or simply need additional wellness support, my goal is to help you feel better without the inconvenience of a clinic or hospital visit.
By bringing evidence-based care directly to your doorstep, I help bridge the gap between traditional healthcare and personalized, accessible wellness services.

Like the monarch and the labyrinth, we are all on a journey of becoming. May these seeds serve as a reminder to trust the process, honor the path, and embrace the transformation that unfolds along the way.
